New Student Registration

After submitting a lottery application and receiving a seat, follow the instructions below to complete your student(s) registration.

Step 1:

  • Complete the online registration sent to your email.

Step 2:

After completing the online form, bring the following items to the front office.

  • Certified birth certificate (no copies)

  • Completed immunization records

Certified birth certificates are available from the Idaho Bureau of Vital Statistics and can be ordered online at www.vitalchek.com. Immunization records are provided by your doctor and can also be obtained at the Southeast Idaho Health Department.

Step 3:

There are two forms required by the State of Idaho to register for school. You will complete them in the front office.

  • State Language Survey

  • Parent Employment Survey

Step 4:

Finally, there are two additional school forms to fill out in the front office.

  • Technology Agreement

  • (Optional) Medication Form (gives the school permission to give bandaids, first aid, medication)

Once all registration documents have been turned in, you will receive a teacher assignment via email.
